I agree that each draw against my credit line is a separate advance of money by Wheeling Island Hotel • Casino • Racetrack. If I receive the advance before I execute a credit instrument, I promptly will sign a credit instrument in the amount of the advance. I hereby authorize Wheeling Island Hotel • Casino • Racetrack in its sole discretion to apply any and all chips, cash, or cash equivalent I may redeem first to the reduction of any outstanding credit balance, with the remainder, if any, to be returned to me.

I authorize Wheeling Island Hotel • Casino • Racetrack, its subsidiaries, affiliates and agents to complete any of the following missing items on these instruments: (1) the name of the payee, (2) any missing amounts, (3) a date, (4) the name, account number, and/or address and branch of any bank or financial institution, and (5) any electronic coding of the above items. This information can be for any account from which I now have or may in the future have the right to withdraw funds, regardless of whether that accounts now exists, or whether I provided the information on the account to Wheeling Island Hotel • Casino • Racetrack.

I authorize Wheeling Island Gaming to (1) verify any information given in this application, (2) obtain any information it believes to be appropriate in connection with this application.

I authorize Wheeling Island Hotel • Casino • Racetrack to investigate my credit report and to furnish information concerning such credit records to credit reporting agencies and others who may properly receive this information.

I agree to (a) to pay all costs of collection, including attorney’s fees, to the extent such costs are permissible under West Virginia law. (b) to waive any requirements of presentment, (c) that the debt for which this credit instrument is issued was incurred to the State of West Virginia, (d) that West Virginia law exclusively applies to this credit instrument and enforcement thereof, and (e) submit to the exclusive jurisdiction of any court, state, or federal, in the State of West Virginia.

I understand that any credit extended may only be used for Slot or Table Game wagering. I understand Wheeling Island Gaming reserves the right to deposit markers at any time without notification.

Do you understand the requirements of the Marker Redemption Policy?

All Markers are expected to be redeemed (paid) before leaving the casino if the guest wins.

Any marker balance unused is expected to be returned before the guest leaves the casino.

The dollar amount of the marker issued determines when you must repay the marker. The marker will be deposited if you have not redeemed it by the deadline listed below and drawn from your bank account. The following are the maximum repayment deadlines. The date of issuance counts as the 1st day:

Markers up to $1,000 will be held until the morning of the 7th day from the date of issuance.
Markers between $1,001 and $5,000 will be held until the morning of the 14th day from the date of issuance.
Markers between $5,001 and $10,000 will be held until the morning of the 21st day from the date of issuance.
Markers above $10,001 will be held until the morning of the 30th day from the date of issuance.

The last marker drawn must be the first one to be redeemed by guest.
