I certify that all information I have provided is accurate and that I am 21 years of age or older. I authorize Valley View Casino & Hotel, an enterprise of the San Pasqual Band of Mission Indians, and their representatives to conduct all investigations deemed necessary for the approval and periodic review of my credit standing using the information provided. This includes, but is not limited to, my Central Credit report which includes outstanding credit at other casino properties, consumer credit report and bank balances. Per the policy and procedures of Valley View Casino & Hotel, a completed and signed application is required prior to the extension of credit. If it is discovered through investigation that information enclosed in this application has been provided falsely, civil and criminal action may be taken.

I authorize Valley View Casino & Hotel to deposit all signed checks into any accounts I have provided on this credit application in the time frame agreed upon on the check. I guarantee funds will be available in these accounts as agreed. If, for any reason, I am asked to leave Valley View Casino & Hotel on a permanent basis, my outstanding checks will be submitted for payment to my financial institution immediately.

In regards to collection; I agree to make all payments within 30 days as agreed. If collection is required, I agree to submit to the jurisdiction of any state or federal court within California. I also agree to pay all costs of collections, including Valley View Casino & Hotel attorney fees and court costs, in addition to any amounts authorized by law. If I have not paid my debit in a timely manner, as agreed, I authorize Valley View Casino & Hotel to use any credit card that I have presented for use, for any purpose. I also authorize the retention of all jackpots, chips or winnings to apply toward the reduction of any outstanding credit balance with the remainder, if any, being returned to me.
