At Soaring Eagle Casino and Resort (hereinafter the "Casino"), we value our customers and take pride in providing them with the most exciting entertainment services available in the gaming industry today. In the course of providing credit and other financial services at the Casino, we obtain certain personal and financial information about our customers that we realize they may consider confidential. We know how much our customers value their privacy, and are therefore providing this notice to explain our policies for collecting and using this information. We collect non-public personal information, including confidential personal information, about our customers from the following sources: Information our customers provide on applications for credit and on other forms; Information about our customer's transactions with us or with other companies; Information we receive about our customers from other sources, including credit bureaus, relating to their history and previous financial transactions. We restrict access to nonpublic personal information we collect about our customers to those employees who need to know that information to process credit applications or provide other products to our customers. We also maintain physical, electronic and procedural protections to guard our customer's nonpublic personal information. We do not disclose any confidential personal information about our customers, former customers to anyone, except as permitted and/or required by law.
I certify that I am 21 years of age or older. Accordingly, I must be 21 years of age or older to apply for credit at the Casino. I am aware that false misrepresentation of my age on this application subjects me to criminal prosecution.
I represent and warrant that I am applying for credit from the Casino for my own personal gaming purposes and that any credit extended to me by the Casino will be used only by me and only for that purpose. I agree that I am liable for the repayment of any and all credit that is extended to me by the Casino regardless of how or by whom such credit is used. I hereby irrevocably and unconditionally waive any claim or defense that I am not liable for the repayment of any portion of the credit that is extended to me for any reason, including, but not limited to, the allegation that such credit was used by, or for the benefit of another person, with or without the knowledge or consent of the Casino.
I authorize the Casino to investigate my credit record and to conduct such other investigations as it deems necessary, and to furnish information concerning my credit record to credit reporting agencies and others who may properly receive this information. I acknowledge that the Casino has not offered or agreed to defer presentment of any check I have given to it or may hereafter give to it, and that Michigan Compiled Laws sections 487.2121 through 487.2173 have no application to any transaction between the Casino and me. I agree that if I fail to pay any indebtedness due to the Casino and the indebtedness is placed with an attorney for collection, I will pay the attorney fees and costs incurred by the Casino. I hereby authorize the Casino, at its sole discretion, to apply any and all chips or slot jackpot winnings I may redeem first to the reduction of my outstanding credit balance, with the remainder, if any, to be returned to me.
Markers are to be paid off at the conclusion of the player's gaming play. Patrons that do not comply with this requirement are subject to the Marker being presented for deposit within 14 days of the Marker being issued and/or possible suspension of credit privileges.
I certify that all of the information provided on this application is true and accurate. I am aware that I may be subject to civil or criminal liability if any material information provided by me is willfully false.
I hereby acknowledge and agree that this Application may be signed electronically and that such electronic signatures shall be deemed an original for all purposes. I represent and warrant that by signing this Application electronically or causing this Application to be signed electronically that I have the authority to do so and that such electronic signature shall be sufficient to bind the respective party under this Application.
