I HEREBY CERTIFY THAT THE INFORMATION GIVEN ABOVE IS TRUE, ACCURATE AND COMPLETE
By my signature below, I authorize JESR, LLC dba J Resort & Casino (J Resort), its employees, agents, representatives, contractors or any third party acting on its behalf, to obtain and verify my personal and business financial information from any financial institution, employer or any other third party, including, but not limited to, bank account balance information, employment and credit score, and to obtain any credit related, gaming related, or employment history information about me, and to report any information to another casino, regulatory body or to a credit bureau agency. I agree not to hold any of the foregoing entities, including J Resort, responsible or liable for the use or release of any such information. I agree that J Resort will retain this application whether or not I am granted credit privileges. Before drawing on my line of credit, if granted, I agree to sign credit instruments (including casino markers, promissory notes or checks) in the form presented to me in the amount of the funds drawn. I authorize J Resort to complete any of the following missing information on these credit instruments: (1) name of the payee, (2) any missing amounts, (3) a date, (4) name, account number and /or address and branch of any banks or financial institutions, (5) electronic encoding of the above, (6) any other information not prohibited by law. I hereby waive any right to place a stop payment on any credit instrument. All credit instruments are due immediately upon oral or written demand.
The information provided in items (4) and (5) above can be for any personal or business accounts from which I have now or may have the right in the future to withdraw funds regardless of whether or not that account now exists and whether or not I provided the information on the account to J Resort. J Resort may obtain payment of any credit instrument from any existing or future bank account. I hereby certify, irrespective of any gambling, debt collection or currency exchange laws in the country in which I reside, that at the time I sign any marker, I have on deposit in accounts on which I am an authorized signatory for all purposes, without restriction, funds sufficient to pay such markers upon demand. I hereby waive to the fullest extent permitted by law any such laws that may otherwise prohibit or limit my obligations as provided herein. To the extent I am unable to waive such laws, I will arrange for payment through other accounts or by other means to legally comply with my obligations hereunder.
I have the ability and intent to legally pay through my financial institution(s) the funds represented by the markers signed by me given to J Resort. Casino credit is intended to be used solely for gaming purposes at J Resort only. I agree each credit instrument is a separate transaction and each term and condition contained herein shall apply separately to each such transaction. If I receive any funds prior to executing a marker or other credit instrument, I agree to promptly sign a credit instrument in the amount of such funds received in the form presented to me.
I agree that any credit granted to me may be suspended or revoked at any time without notice in the sole discretion of J Resort, and I hereby waive and release any and all claims for damages, claims, losses (including gambling losses) or expenses that I may incur as a result of such credit being suspended or revoked.
I agree that Nevada law exclusively governs the terms and conditions of my casino credit line, advances, or credit instruments. I agree that J Resort and I will litigate any disputes involving the casino credit line, the debt, the markers, or the payee in any court, state or federal, in the State of Nevada. I submit to the jurisdiction of any court, state or federal, in the State of Nevada and waive any objections to the jurisdiction of the same or any requirements of the presentment of any credit instrument. I agree to pay all costs of collection of the credit line, the debt, casino markers, and any credit instrument including attorney's fees, costs, and interest at 18% or the highest statutory amount permitted from time to time by Nevada law, in addition to any other amounts authorized by law.EACH PARTY HERETO WAIVES ANY AND ALL RIGHTS TO HAVE ANY CLAIMS OR CONTROVERSIES ARISING HEREUNDER TRIED BY A JURY.

The decision to extend any credit at any time and from time to time is in the sole discretion of J Resorts. I acknowledge that J Resorts extends credit without regard to race, religion, ethnicity, gender or sexual orientation.
WARNING: For purposes of Nevada Law, a credit instrument is identical to a personal check and may be deposited in or presented for payment to a bank or other financial institution on which the credit instrument is drawn. Willfully drawing or passing a credit instrument with the intent to defraud, including knowing that there are insufficient funds in an account upon which it may be drawn, is a crime in the State of Nevada which may result in criminal prosecution in addition to civil proceedings to collect the outstanding debt.
