There is incorporated into this Application the provisions set forth below on this application. I have read and approve all of the provisions of this Application.

1. I hereby authorize the Casino, its officers, employees and outside credit verification and reporting services designated by the Casino to contact, verify and evaluate (a) all of the statements contained in this Application, (b) all bank references relating to the Applicant, and to obtain general references and credit reports relating to the Applicant as the Casino deems necessary. All of such information obtained shall be available to the Casino and to any debt collector or attorney designated by the Casino to recover and debt hereafter owed by the Applicant to the Casino. I expressly waive any right of privacy and confidentiality relating to all information and data obtained by the Casino, its officers, employees, outside credit verification and reporting services, and attorneys and expressly release said parties from all liability of any kind or nature.

2. I certify that all statements made in this Application are true and complete and that the Casino will rely upon the statements in this Application. I further understand that any misrepresentation in this Application will be sufficient cause to deny credit and may give rise to legal rights.

3. Should the Casino extend me credit and should any check cashed on my behalf by the Casino not for any reason be honored and immediately paid in full by the financial institution upon which it is drawn, I agree to pay, in addition to any costs, fees or damages provided by law, reasonable attorney’s fees and costs uncured by the casino to recover sums due to it.

4. A portion of California Civil Code Section 1719 is made part of the Application. I acknowledge the provisions thereof including the costs and damages provided therein.
