I give my permission to Bally’s Shreveport Casino & Hotel, or any of its affiliates, to investigate my credit history by making inquiries with the financial institutions listed, subsequently discovered or any consumer credit bureaus. I do hereby consent to your disclosure of information from time to time to any credit reporting company concerning any extension of credit from you to me, my payments or failure of making payments on same and any other information pertaining thereto, and I hereby waive any right to notice of you making such disclosures. I will not hold these financial institutions or you responsible for any information released. In the event a legal action is brought to collect any amounts owed, I agree 1) To submit to the jurisdiction of any state or federal court in Louisiana; 2) That said action shall be governed by the laws of the State of Louisiana; 3) I agree to pay interest on all amount due by me at the annual rate of eighteen (18%) percent from the date's when such amount first became due until the date of final payment.
